CONCORDIA HEALTH PLANS - BE WELL REWARDS PROGRAM
Like last year, the 2013 Be Well Rewards program includes the opportunity
to earn up to $50 in bonus points. However, this year there will be four seasonal
bonus opportunities—each worth 25 points. You can complete any
two throughout the year to earn your $50 bonus. Pay close attention to the
dates of each activity—each is only available during a certain time period
(and must be recorded in that timeframe), so plan accordingly!
